Looking at that, your down swing - 15 seconds to 19 seconds - and contrasting it with your back swing - the first 08 seconds - is very illuminating.
If you can align your back swing - which is far too flat and dragged back in behind you, from my perspective - closer to your down swing then greater consistency and better results might be forthcoming.
It might help to break your tendency to drag the club back in and behind you, if you place your golf bag, upright, about one yard to the right of you with the back of the bag in line with your heels.
Best done in slo-mo till you get used to it.
